How do I create a Review-Form in HTML/PHP?

I have been looking for this a very long time as I need it for a site I have.

Look on the page here: http://www.tsfz.org/store1/index.php?route=product/product&product_id=50

Then tab to "Review" and I need an almost exact function as that one (Maybe with a more line of text though)

There is almost no information about this on the net.

(this function uses moderation so I dont think copying works..)
